With just one call, you can access private duty, skilled home care, disease state management programs, TeleCare, palliative care, home hospice and residential hospice services through the Keystone family of agencies … all with a 24-48 hour start-of-care commitment.

As your needs change, the same team you’ve come to trust with your comfort and security stays with you to manage your health at every
stage. Care emphasizes empowering persons living with HIV/AIDS and families to make care decisions; respecting each individual’s choices; and developing plans tailored to an individual’s specific needs.

Persons living with HIV/AIDS have the right to competent care, control over their wishes, and an environment of respect and dignity. Keystone’s team of healthcare professionals — nurses, therapists, social workers, home health aides, and homemakers — provides services to manage the full spectrum of physical and emotional needs of persons living with HIV/AIDS.

Needs vary, and depending on the type of assistance required, services are provided under these programs:

Skilled home care is provided after a hospitalization and/or when a person living with HIV/AIDS requires care and support for any of the following:

• Changes in medical condition
• For assistance with daily activities, such as bathing, walking, eating and companionship
• For multiple medication changes, wound care and rehabilitation
• Conditions requiring skilled nursing intervention, such as deterioration in a medical condition.
