InTouch

Keystone InTouch preserves the independence and freedom of those who can still count on themselves to manage most of day-to-day life but who may have a condition that needs medical supervision. We use a combination of technology, communication devices and periodic staff contact to monitor patient health and well-being in the comfort of their own home.

Visiting nurse case managers supervise a response team that can react to up-to-date, accurate health status information provided by our monitoring systems. This permits independence and privacy when our patients are stable, and enables timely interventions when their health situation changes.

Keystone InTouch Helps with a Light Touch

  • Monitor vulnerable individuals and those with chronic health issues
  • Manage adherence to medication
  • Identify changes in health status
  • Control chronic conditions
  • Break the cycle of hospice readmissions
  • Promote healthy independence and quality of life
