Independent Living Programs
Keystone InTouch offers custom programs based on individual
health status, medication needs, residential context and family
situation.
The Independence Program
The Independence Program uses passive monitoring to analyze the
activities of daily living. If there are significant changes in a
patient's daily pattern, a Keystone nurse case manager is
alerted.
The Medication Management Program
The Medication Management Program uses the MD2 Personal
Medication Dispenser to remind patients to take their medications
and to dispense the appropriate dose at the correct time of the
day. In the event of a missed dose, the MD2 sends an alert through
a secure standard telephone line to a Keystone nurse case
manager.
The Disease Management Program
The Disease Management Program monitors vital signs with the use
of a wireless modem that connects to a patient's own glucose meter,
blood pressure cuff, weight scale, spirometer or pulse oximeter.
Readings are transmitted through a secure server to home nurse case
manager for monitoring in between routine RN visits.
