Home Health

Home Health is designed for patients who have been recently hospitalized, have chronic conditions, or for those who need assistance with daily activities or require skilled nursing care.

With the highest level of comprehensive care available, our home health team works with patients, their families, and physicians to create a restorative in-home treatment plan that promotes health and healing. These services are available:

  • Post-hospitalization care
  • Help managing chronic illness
  • Assistance with daily activities
  • Medication monitoring, adherence, and changes
  • Physical rehabilitation
  • Wound care
  • Patient education on disease and self-care

Home Health Team

Registered Nurses

Highly skilled registered nurses provide individualized patient care. In accordance with physician’s orders, KeystoneCare nurses assess the patient’s physical condition with special attention to promoting health and healing at home.

Physical/Speech/Occupational Therapists

In coordination with the patient’s physician, specialized therapists will assess the needs of home-based patients and develop treatment plans. They will work with the home care team and referring physician to provide therapy services as defined in treatment plans.

Social Workers

KeystoneCare’s team of social workers provide support and counseling to address the psychosocial needs of the patients, families and caregivers.

Certified Nursing Assistant, Home Health Aide, and Homemakers

This team is specially trained to provide personal care to patients such as skin care, bathing, shampooing, shaving, and any personal grooming needs, as well as food preparation or light housekeeping.

Trained Volunteers

KeystoneCare’s trained volunteers are dedicated to serving you, your loved one, and your family. Volunteers are skilled in active listening, and are sensitive to the needs of patient, caregiver, and family, and may help in any of these ways:

  • Companionship
  • Caregiver respite
  • Reading to the patient, writing letters or playing a game
  • Assisting at mealtime

All of KeystoneCare’s professional staff are fully licensed in the state of Pennsylvania. KeystoneCare conducts thorough background screenings, including Pennsylvania criminal history and child abuse clearances.
